We accept GP and specialist referrals for all our clinics. Referrals are triaged based on clinical need.

 

To refer a patient, please complete a detailed referral letter or complete our referral form (download here) and securely fax to 03 8677 2494. Any additional information such as any recent x-ray or pathology results would be greatly appreciated.
